SLS Ballast Issue

I have a Dual 250 watt (M58) SLS (Blue Wave) Ballast and can not find any info on them on internet. I find all other ANSI coded ballasts but not the M58. One of my MH is not working and feel like one of the ballast went out on me. I tried to see if there is a fuse somewhere on it, but no luck.

How can i fix this, it is out of warranty. Any links would be great.[/img]

Matt beat me to it. Def. give them a shout. They seem to be pretty good w/customer service (IME).
