Overflow

I just got a tank and need an overflow for the back. I want it to be calfo style. The tank used to have one, but was not included when I bought it. Any idea where to get this? I can find glass pretty easy, but where can I get the notches cut in the top? I'm new to this, so if I didn't describe this right, let me know. I need it like this picture, but from one end of the tank to the other (75g tank, so 48")
