Clown fish death question.

Hi,
I'm a newbie here. I have fat and healthy false percula clown that has been eating like a pig 'til last Friday. The next day, it looked so skinny like it's been starving for months; the white part on the head looked somewhat sunken in some spots (sorry, no pictures). It just died this morning. Does anyone know what might be the cause?

Thanks.

Hi Matt,
The best description I can give about the sunken spots on the white bar at the head is like hole-in-the-head symtom but you do not see the flesh, the skin is still there. Once the fish got skinny, it stayed in one place with labored gill movements, swam in circle once in a while but it still went atfter the food when I fed. I've had the fish for about a month with a bicolor blenny, one pepermint shrimp, a few blue leg hermit crabs and a cucumber (it does look unhappy though). I've added a much smaller false perc about 10 days ago and it looks healthy so far. If something happens to this one, I'll make sure to get a picture of it. I tested the water on Sunday and the parameters are normal.
Thanks.
